By booking an appointment and receiving services from Face Creations by Marilyn, you acknowledge and agree to the following:
I understand that facial treatments, acne treatments, anti-aging treatments, HydraFacial treatments, hydrodermabrasion, oxygen facials, microdermabrasion, chemical peels, dermaplaning, microneedling, nano infusion, BioRePeel, coral peels, LED light therapy, high-frequency treatments, waxing services, brow lamination, brow tinting, lash lifts, lash tinting, makeup applications, Botox, neurotoxins, injectable treatments, and other aesthetic services may involve certain risks and potential side effects.
Potential side effects may include, but are not limited to:
• Redness
• Swelling
• Tenderness
• Bruising
• Dryness
• Peeling
• Skin Sensitivity
• Temporary Irritation
• Allergic Reactions
• Breakouts or Purging
• Hyperpigmentation
• Hypopigmentation
• Infection
• Scarring
• Burns
• Asymmetry
• Unsatisfactory Results
• Other known or unforeseen complications
I understand that results vary from person to person and that no specific results or outcomes can be guaranteed.
I certify that I have disclosed all relevant medical conditions, medications, allergies, skin sensitivities, recent cosmetic procedures, and health concerns that may affect my treatment.
I understand that it is my responsibility to follow all pre-treatment and post-treatment instructions provided by Face Creations by Marilyn.
I acknowledge that failure to follow recommended aftercare instructions may increase the risk of complications and may negatively affect my results.
I voluntarily consent to treatment and assume all risks associated with the services I receive.
To the fullest extent permitted by law, I release and hold harmless Face Creations by Marilyn, its owner, employees, contractors, affiliates, and medical providers from any claims, injuries, damages, losses, liabilities, or expenses arising from services provided, except where prohibited by law.
I understand that all services and product sales are final. No refunds will be issued for services rendered or products purchased.
