By booking an appointment and receiving services from Face Creations by Marilyn, I acknowledge and agree to the following:

I understand that facial treatments, acne treatments, rejuvenating facials, hydrodermabrasion, oxygen facials, microdermabrasion, chemical peels, dermaplaning, nano infusion treatments, BioRePeel treatments, LED light therapy, high-frequency treatments, waxing services, brow lamination, brow tinting, lash lift and tint services, and makeup applications may result in temporary skin reactions and individual responses that vary from person to person.

Possible Temporary Reactions May Include:

• Redness
• Swelling
• Tenderness
• Dryness
• Flaking or Peeling
• Skin Sensitivity
• Temporary Irritation
• Allergic Reactions
• Temporary Breakouts or Purging
• Temporary Changes in Skin Pigmentation
• Unsatisfactory Results

I understand that results vary from person to person and that no specific results or outcomes can be guaranteed.

I certify that I have disclosed all relevant medical conditions, medications, allergies, skin sensitivities, recent cosmetic procedures, and other health-related information that may affect my treatment.

I understand that it is my responsibility to follow all pre-care and post-care instructions provided by Face Creations by Marilyn.

I acknowledge that failure to follow recommended aftercare instructions may affect my treatment results.

I voluntarily consent to receive the services selected and understand the potential temporary reactions associated with cosmetic skincare treatments.

To the fullest extent permitted by law, I release and hold harmless Face Creations by Marilyn, its owner, employees, contractors, and affiliates from claims arising from services provided, except where prohibited by law.

I understand that all services rendered and products purchased are final. Refunds are not provided for services rendered.

All services provided by Face Creations by Marilyn are cosmetic in nature and are not intended to diagnose, treat, cure, or prevent any medical condition.
